Little Sprouts Summer Day Camp Terms and Conditions

Registration

Registration is on a first-come, first served basis. Early registration helps to ensure availability. Sprout and Blossom reserves the right to substitute speakers, presenters, and/or any program to be offered at the Event at any time. By registering for this Event, Customer represents, warrants and ensures that the participant is physically and mentally able to participate in the program for which he/she is being registered and/or any others in which he/she may participate. Customer understands that this registration is non-transferrable and non-assignable and that any other person(s) wishing to attend the event must register in their own name(s). Billing and Payment

Billing and payment processing for Sprout and Blossom events is performed through WIX. Payment is due upon registration, in full.

Customer's 
Right to Cancel

If you wish to cancel your registration, you may cancel by emailing sproutandblossom@outlook.com or calling (909) 658-5999. 100% refund will be issued if requested by June 1st, minus a $35 administrative fee. After June 1st no refunds for camp registrations will be issued. We do not prorate the cost if you miss some days.

Release

In consideration for being accepted by Sprout and Blossom to participate in the below described program, we (I) [and on behalf of our (my) child-participant] do hereby release, forever discharge and agree to hold harmless Sprout and Blossom/The Blessing Center and the staff thereof from any and all liability, claims or demands for personal injury, sickness, or death, as well as property damage and expenses, of any nature whatsoever which Sprout and Blossom Little Sprouts Summer Day Camp. Registration and Release Forms may be incurred by the undersigned and the child-participant that occur while said child is participating in Sprout and Blossom Summer Program .Furthermore, we (I) [and on behalf of our (my) child-participant] hereby assume all risk of personal injury, sickness, death, damage and expense as a result of participation in recreation, including tree climbing, and/or work activities involved therein. I authorize the staff of Sprout and Blossom/The Blessing Center to act for me according to their best judgment in any emergency requiring medical attention. I understand that I will be financially responsible for all charges and fees incurred in the rendering of said emergency treatment, regardless of whether or not my medical insurance would cover such charges and fees. I have no knowledge of any physical impairment that would be affected by the above-named camper’s participation in the camp program as outlined in the brochure.

Photo Waiver and Release

- I hereby authorize Sprout and Blossom/The Blessing Center and those acting pursuant to its authority to photograph, video tape, or use any other electronic method of recording the participants’ likeness and/or voice to be used at Sprout and Blossom/The Blessing Center’s publications and/or web sites. The photographs and/or video footage will not be digitally manipulated to change its content. -I hereby give Sprout and Blossom/The Blessing Center the absolute right and permission, without restrictions, to make, copyright, and/or use, re-use, or publish said photographs/video footage of the participant in which he/she may be included in whole or in part, and waive any right to inspect and/or approve the finished printed materials, videos and/or web sites where my image appears. -I waive any right to compensation for the participant’s appearance in these printed documents, videos, or web sites in any and all future uses of the photographs and/or video footage. - I have read and fully understand the terms of this release.
